DEFINED HEALTH & FITNESS – MEMBER ACCESS & LIABILITY WAIVER

IMPORTANT: READ CAREFULLY BEFORE SIGNING. THIS IS A LEGALLY BINDING AGREEMENT.

By signing below, I acknowledge and agree to the following terms regarding my membership and access to Defined Health & Fitness (“DHF”):

1. Authorized Member Access Only
Access to Defined Health & Fitness (including cardio, weight, and functional fitness areas) is strictly limited to active DHF members who have purchased a key fob. Each member must use their personal, assigned key fob for entry. Sharing key fobs is prohibited — including with family members — even if I am present.

2. Access Areas
Entry is permitted only to the main gym room and locker rooms. Entry during staffed hours must be through the front entrance.

3. Unstaffed Hours & Assumption of Risk
I understand that I may access DHF during unstaffed hours using my key fob. I acknowledge there will be no staff supervision or emergency assistance available during those times. I voluntarily assume all risks associated with using the facility alone. DHF highly recommends training with a workout partner who is also a key fob member, but this is my personal decision.

4. Health & Liability Waiver
I certify that I am in good physical health and capable of participating in exercise activities. I release and hold harmless Defined Health & Fitness, its owners, staff, and affiliates from any and all liability for injuries, illness, accidents, or damages arising from my use of the facility, whether occurring during or outside of staffed hours.
